Clinical Analysis of Choledochal Cyst


Abstract
Cystic dilatation of the biliary duct, or choledochal cyst is an uncommon disease entity. Since Vater first reported a case of this disease in 1723, more than 1500 cases have been reported in the world's literature.
Author reviewed 33 patients of choledochal cyst treated an managed at the Department of General Surgery, Chungnam National University between 1966 and September 1991.
@ES The results were as follows:
@EN 1) 27 cases(89.6%) were below 30 years of age and the female to male ratio was 5.6 : 1 which revealed predominant in female.
2) Frequency of each symptom in classic triad was abdominal pain(90.9%), abdominal mass (69.7%), and jaundice(39.4%) respectively.
3) According to Todani's classification, type I was noted in 28 cases, type IV in 3 cases, type IV in 1 cases an type V in 1 case.
4) Alkaline phosphatase was elevated in 18 cases(54.5%) and bilirubin in 13 cases(39.4%).
5) Preoperative diagnostic accuracy was 84.9%.
6) The surgical procedures were excision of cyst and Roux-en-Y hepaticojejunostomy in 14 cases and choledochocystojejunostomy in 14 cases.
7) The biliary stones associated with cyst were accompanied in 10 cases(30.3%) and 2 cases of cyst-associated cancer were presented.
8) The postoperative complications were occured in 10 cases with ascending cholangitis in 5 cases, wound infection in 4 cases, anastomotic leakage in 2 cases, anastomotic stricture in 1 cases, and atelectasis in 1 case.
